How do you ask a girl out that you haven't seen for two months?

I have been out of high school for two months and recently got the number of a girl that i've had a crush on. I am 95% sure that she wanted to go out with me too when I last saw her, but it wasn't meant to be. It's been two months since I last saw her, how do I resurrect what we had and how should I go about it? Should I flat out ask her out? Should I call or text? I've never asked someone out before.
